All:

I am in the final stages of my dedicated home theater, and need to choose on the door this week. My room is 12X20, and I have a 3X3 window done using UPVC. For the door, I am thinking of a flush door (Plywood), OR a solid wood door. More than the looks or the finish, I would go for a better sound proof door.

Pls suggest what would be the best option ?

Your biggest leakage of sound would be the window, try and block it off. You don't need a window in a dedicated ht. For doors, the thicker the material it's better. Double doors are excellent.

You can have a very good door but it will not serve the purpose of you have air leakage. Sound will escape from the path of least resistance. If you do get a solid door, make sure to seal all the edges with airtight gaskets. A door made with multiple layers of Plywood, glued to each other will work as well as a slab solid door.
